PEA Business Awards 2013 Entry Form

To nominate an individual or team leader you consider worthy of winning any of the awards, please complete this form. (Entrants may nominate themselves.) One further document with information to support the entry may be attached, but must not exceed more than 500 words. For further guidance refer to the category descriptions. Choose one or more award categories your organisation would like to enter from the full list of categories, but please complete a separate entry form for each category entered.

Entries should contain specific commercial data and screen grabs/URLs and passwords where applicable and should be endorsed by the company's chief executive. Each submission should contain a high resolution print quality (300dpi jpeg) head and shoulders photograph of the nominee(s).

Entry to the PEA Business Awards is free.
